We are currently recruiting for a proven and experienced Admissions Coordinator (long term care experience in Admissions required) for our skilled nursing facility we own and operate in Memphis, TN

Position: Admissions Coordinator

Summary: Manage the patient admission process during the first 72 hours

Essential Duties & Responsibilities:
• Manage and analyze patient care during first 72 hours following admission/readmission
• Conduct Initial Nursing Assessment for newly admitted or readmitted patients to establish solid physical and functional baseline data
• Instruct unit staff on patient choices, care, and documentation
• Complete the Interim Care Plan
• Validate admission orders with patient’s physician
• Identify and document reasons for skilled care and response to care
• Lead by example to promote and guide teamwork and hospitality
• Involve families in admission process and group meetings
• Track data specifically identified to evaluate patient care and process outcomes during first 72 hours in facility (falls, discharges to hospital, family/patient feedback)
• Use customer satisfaction and employee surveys to implement changes in the admission process
• Participate in marketing the facilities expertise in Clinical Care, Rehabilitation, and other specialty programs where available
• Initiate discharge planning process with Social Services
• Obtain ordered or needed equipment or referrals.
• Participate in Full Life Conference
• Provide MDS Coordinator with baseline assessment data
• Transition care of the patient from Admission Team to Unit Team
• Develop case studies and hold Team Conferences to explore successful strategies or improve the process
• Other special projects and duties, as assigned.

Job Requirements:
• Experience in physical and functional adult patient assessment
• Possess working knowledge of Medicare, Medicaid and Managed Care coverage, referral, and reimbursement methods
• Minimum of three (3) years related experience.
• One (1) to three (3) years related experience; supervisory experience preferred.
• Effective verbal and written English communication skills.
• Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
• Customer service oriented with the ability to work well under pressure.
• Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
• Strong analytical and problem solving skills.
• Ability to work with minimal supervision, take initiative and make independent decisions.
